Why community-based and home-based palliative care—and why now? Health care reform and value-based medicine are driving changes across the health care continuum and making the availability of palliative care in home-based settings a cost-effective and quality-driven imperative. While palliative care is an unspoken core of home-based primary care, it is time to formally integrate the core principles of palliative care into primary care practice—using interdisciplinary teams, ensuring advance care planning and setting goals of care, and pain and symptom management. This session will provide an overview of community-based and home-based palliative care, describing the foundational principles that shape home-based palliative care delivery models. The benefits for patients, providers, and payers of embedding palliative care in home-based primary care systems will be discussed. The session will illustrate why person-centered, home-based palliative care is the future of quality medical care for the sickest and most complex patients and their families.
