Learn how a VA home-based primary care setting in Southern Nevada developed a successful skin and wound care program in which all members of the interdisciplinary team—including the provider, case managers, physical therapist, dietitian, social worker, pharmacist, and psychologist—play a crucial role.

• measure the effectiveness of the preventive measures put in place by the team
• develop PICO questions that reflect the goals of a skin and wound care program
• list five interventions taken by the interdisciplinary team to grow this program.
